Dear Sir,

I, Davendra Swarup Agarwal, S/o. Late Shri K.S.Agarwal retired after completing
58 years of age from M/s. Ipca Laboratories Ltd.,142-AB, Kandivli Industrial Estate,
Kandivli (West), Mumbai at their Lucknow depot on 31st of January, 2008.

I, immediately submitted forms duly filled and signed with photographs etc. to my
P.F and monthly pension to your office. I had received P.F.settlement vide your office
letter no. 70202935/cash/2008-2009/Mal 45/ 28/208/R.L.no.101 dated 10.5.08 directly to
my Canara Bank,Hazratganj, Lucknow, crediting to my SB.A/c no. kgsb 000026007.

As regards to my monthly Pension M/s.Ipca Laboratories Ltd., had submitted duly
filled P.F.forms to your office on 17.11.08 under Form 10 D.

Since a long time has been passed and I am still waiting for my monthly Pension. I
am in accute shortage of money.

I would, therefore request your goodself to look into the matter and start my monthly
Pension at the earliest.
