We are perturbed to note that my ULIP Policy as above is reported to have been terminated after 2008 due to non payment of annual premium @Rs.3000/- though we remitted payment regularly in line with the terms of the Policy as below {Scanned copies of the money receipts (four nos.) attached for your ready reference}.

The above fact came to our notice on date during our visit to your office at AD-55, Salt Lake City, Kolkata – 700064 for remitting payment of Rs.3000/- towards premium for 2011-12 and we got surprised to note your lackadaisical attitude towards updating record resulting in financial losses, harassment, mental anxiety, tension etc. which is totally unwarranted.
We, in view of above, would request you to pl. enquire into the matter and inform
i) How the policy got terminated inspite of receipt of premium regularly?
ii) How & when it would be regularized?
iii) How would you compensate financial losses to this account?
iv) How would you compensate towards such undue harassment, tension, anxiety all together to this consumer?
