I, Munaf Iqbalbhai Gori was working with Max New York Life insurance as an Agent (Agent Code-394933)
and my License expired on 20/09/2013 and my License No. was 8292452.

Now I have applied for fresh License in LIC of India and already passed IRDA examination for becoming Agent. But my PAN No. which is APZPG0942B is attached with Max New York Life insurance and because of that I am unable to work with LIC of India. I tried to get solution from Max New York Life insurance but they are not replying.

So, I request you to do needful and cancel my PAN No. from Max New York Life insurance so that I can start my work as an Agent with LIC of India.
