I have purchased prepaid simcard No.9989175689 on 19th April, 2010 from M/S. KINGS MOBILE, Opposite FAZAL HOSPITAL, Muradnagar, Hyderabad-500028 and submitted the form along with the Copy of Photo ID Card issued by Election Commission /Address proof/ & Photograph as demanded.
