My mother visited to CMC vellore in 3rd sep-12 in gastroenterology and hepatology department with Hospital Reg. no – 223809F, name – gayotri bhuyan. After treatment they are supply to me a medical report and all original invoice / voucher.But they are not supply me any prescription or admission advice slip. Due to this I am unable to submit my medical reimbursement bill in my company. Various time I am communicated with hospital through mail and phone for supplying xerox copy of this document . But they are given me a certificate that they are unable to supply this document. Now they are not response my mail also.

Due to this I am unable to submit my medical reimbursement bill in my company. I am loosing approximately rs. 12000/- ( rupees twelve thousand)

I want photocopy of my all documents.
