I would like to lodge a complaint against Birla Sunlife Insurance Company for forged signature on my medical report acceptance and dumping policy with higher premium for faulty medical report (diabetic)when,I am not.Now I am getting a loss of Rs.25000+mental stress due to higher premium I got less units with higher NAV and also the policy was delayed.
They have done forged signature without giving me intimation about my medical report.The policy docate was directly dispatched to me.I was not in India during that period.The signature is not matching with my signature on my application form.I have done several correspondance with the company but the answer was not satisfactory.
I am here with enclosing all the correspondance done with the company for your kind perusal.Hoping your justice on this subject at your earliest.

Dear Sir,
I have read your communication but I am surprised to note that there is nothing about the forged signature which your company personal has done, which is a very very big offence. The policy is accepted without my permission.
It will be appreciated from Birla Sunlife what are the steps taken for this type of Forged Signature. Awaiting your Reply on this special type of case.

Dear Mr. Sawant,
We refer to your communication dated September 22, 2009 regarding the
captioned policy.
Please note that the aforesaid policy is a Gold Plus Plan with a paying
period of 3 years and benefit period of 8 years. Your policy is registered
for an annual mode of premium payment of Rs. 50,000/- in our records.
We wish to inform you that the policy is issued as per the application form
and illustration submitted by you at inception.
We provide a freelook period whereby you get an opportunity to discontinue
your policy or review your policy features. The same is available to you
for 15 calendar days from the date of receipt of the policy contract. In
such case, the policy fund value plus all charges levied till date
(excluding the fund management charges) will be paid to you and the
contract will be rendered null and void. Since your policy has crossed the
freelook period we are unable to accede to your request for changes in the
policy.
We would like to inform you that the plan offers the option to reduce the
premiums from the 2nd policy year onwards. You will be required to give a
request for the same one month prior to the due date. The renewal premium
can be reduced to a minimum of Rs. 10,000/-. On reduction of the premium
the sum assured chosen at inception remains constant and you will be
covered for the entire tenure of 8 years even after reducing the premium.
Please note that the next due date is December 14, 2009 for an amount of
Rs. 50,000/-.

Complaint against Forged Signature

in Investment.
Dear Sir,
As replied by yourself, I have checked my policy documents the rating acceptance documents were signed on date
03-12-2007 (which is not my signature) as you can verify from my application form.I was not in India during that
period the passport proof can be produced to you if required. I was surprised by seeing my forged signature and my
medical document report showing the abnormality when I am not a diabetic patient at all, because of this my policy is
under rating, I have to pay simply more premium and also received less units against it after paying my first
installment of Rs 50000/- on 25-11-2007 I was given units as on 14-12-2007 basis.
So I request you to solve my case otherwise I will have to suite the case to get me justice. So I request you to
verify properly and reply me at your earliest.

blood sugar. The rating was accepted by the you and based on the same the policies was issued at extra rates on

Dear Sir,
I am holding Birla Sunlife Gold Plus policy No: 001308498 Client No: 0943506853
( Malad Branch) issued on 14-12-2007 on Underwriting Extra (Class) -3. I want to put to your notice that when I got
the policy at that time I was NRI so I could not get the medical reports. I got the blood check up by your
representative in the morning when I have already taken Tea before. After some days I went abroad.
Now as I am in India I have verified my policy and in the blood report it shows Glucose (fasting) 113.00 mg/dl and
Triglycerides 242 mg/dl. This means that I am diabetic. But having shown to my family doctor I am not diabetics, if
required I can produce the fresh blood report if needed.
Now as my policy is Underwriting Extra (Class) -3. I have to pay more premium of policy, when I am not a diabetic
patient. I got less UNITS at the starting and my policy was delayed causing NAV increase. Whereas the payments of
premium was cleared by my bank on 25-11-2007 of Rs 50000/-
I have paid both my & wifes second premium of Rs. 50000/- (Maximiser Fund)each on 5-12-2008 in which I got only
5798.347 Units as compared with my wife policy No.001308517 her units was 6454.869 Units. My wifes birth date is
7-11-1962 & my birth date is 19-9-1963. This investment is totally loss of mines even though I am medically fit.
So I request you sir, kindly give me justice in this matter and adjust my units accordingly and give me the correct
no of units which I should get.
