Mr. Rajan Patil HDFC life approached us based on our net request and provided health insurance booklet and constantly followed up for premium amount.He got the forms signed and collected premium from my son (Amit Bhadakal)after ascertaining from his office and confirming that the policy will be issue in a week.He did not give us a copy of the form filled by him and signed by us.His contact details are as under. On 12-9-2014, Approval was given for Ashok Bhimarao Bhadakal at standard premium and approval was given for Shashikala Ashok Bhadakal with additional premium of Rs 1613 ( incl taxes) Mr.Rajan Patil requested for reimbursement of payment of this additional premium in his HDFC bank personal account and accordingly this amount was paid by cash in his HDFC Ac no 50100032117198 We did not get Policy document inspite of Several reminders via SMS/e mails to Rajan Patil. Further a medical emergency (Dengue )arose for Ashok Bhimarao Bhadakal on 16th Oct 2014 which was communicated to Mr.Rajan Patil by e mail/sms and telephone. All the details are available with us to proove that Mr.Rajan Patil has deliberately fooled us and avoiding us so that Ashok Bhimarao Bhadakal don’t get the reimbursement of hospital expenses of Rs.45000.
