I, Rekha jain d/o sh. Ashok kumar jain r/o 143-a, street no.5, new lahore, shastri nagar, delhi-110031. Sir my pan is acypj3622m. The same was used by another person in agra and these people issued a irda license with insurer name is national insurance co. Ltd. (dist. Agra, uttar pradesh) against my pan detail as fraudulent (the detail as per attachment) in the irda license my father name were changed and name and date of birth was same. These people running for the same from 2012. I have not apply any irda license any kind of time. You are requested to please investigate for the same and take legal action against these people were fraud against us. I have born in delhi and up to till date residing in delhi and no any other address in india. Once against you are humble request you please take the action for the same with earliest possible and solve the situation.
