I ,S .Chandrakanth advisor code 00365708 & licence no757 3278509 hereby declare that i have submitted the resignation and request for cancellation of Agent and Irda codes pertains to ICICI PRU on 26/11/2013 at Kurnool Branch (A.P) by visiting personally and submitted all the belongings of ICICI PRU and confirmed the policies soured in my code by signing in Annexure and also submitted request for Issue of NOC. but so far that was not processed still the advisor code is displaying in IRDA site. Iam requesting you to kindly process my request at the urgent basis and delete my Agent and Irda code & issue me NOC trough mail or hard copy please process my request as this is related to my employement (career) issue
