Dear sir,
I would like to lodge my complaint against harassment for my medi claim and imposed such clause which is not related with my submitted documents and I also inform it to the insurance company as well as to the TPA.
The TPA is M/s. Medicare TPA Services(I) Pvt. Ltd., situated at 6, Bishiop Lefroy Road,Kolkata-700 020 who are now harassing and refusing my claim as on my submitted documents.
You are therefore, requested to verify the submitted documents as they have imposed under clause of 4.3 which I have been protesting against their intentional imposed clause and not to relate with submitted documents.

Please verify my documents and their imposed clause of 4.3.
