I( Mr. Jai Parkash) have opted a policy on 15thJune2010 from TATA Through Agent SMC INSURANCE. But till the date I have not received my policy document but I know my policy number C213409809 as told by TATA and SMC people. Till july I was asking for my policy documents from my agent SMC INSURANCE and TATA LIFE INSURANCE. But they were unable to give me my policy document. When I gave them a request for duplicate policy document they declined my request and after that I applied for free looking of the policy then they decline that also. A brief description of this case is given here :-

1) I applied for a TWO policy on 15thjune2010 Through SMC in TATA By the name of Jai Parkash and Ritu

2) In the month of july I have received welcome letters from TATA and SMC and the policy numbers was mentioned on those letters. To my surprise the name of my wife Ritu was wrongly written printed on that letter and immediately I told my agent in SMC regarding this mistake on this he assured that name will be corrected on the policy document. I was satisfied with the reply and I was very happy about my decision to take such a good policy from such a good brokerage house SMC. But when I received the policy document of my wife Ritu , again it was found that name of my wife was again wrongly printed on the policy. I again told SMC and TATA people about this mistake they did not bother about my complaint and said we can’t help you in this matter. So I got this policy free looked with in free looking period. One more thing when I received the policy of my wife I found there was some miss commitment in policy bond. As per Agent SMC I was getting a Guaranteed returns after 6th year onwards which is of 3.1percent of sum assured and after 10th year I will get 11.2 percent of sum assured and after 20 year I will get a return of 14.3 percent till the age of 100 years. But when I saw policy document there was written that these returns are non Guaranteed returns and guaranteed returns are only 5 % till the age of 100 years. All my happiness was vanished as these two companies are doing misselling and fooling their customers with wrong commitments. In the mean time I asked for my policy document from my agent but he again give me vague answers and told me to wait for few days. I was in very much tension and was very disturbed. I thought I will never get my policy and the behavior of SMC and TATA people was not at all professional and they were giving wrong commitment as well as wrong information to all the customers.

3) But in the month of August 2010 my agent SMC told me that as per TATA company records policy document has already been delivered to me. After that TATA suggested me to apply for Duplicate policy bond but after that they themself declined my request by saying that my policy documents have already been delivered.

4) After this someone suggest me to do complain in TATA and tell them to refund my premium money. Then I complained in TATA and demand for my own money but they refused to do so and send me a photo-copy of POD from Blue Dart courier. When I saw that copy I found that there was my forged signature as you can compare the signatures from copy of application form attached with this letter. You can also find that there is forged signature on English also but I never do signature in English. After this I gave many calls to SMC and TATA and told them to cancel my policy and send me the premium money back to me and send them letter also but till today I have not received my money.

5) Kindly help me and do me the needful.

6) For your convenience I am attaching a scan copy of my application form, complain letter, Regret letter by TATA and courier copy receiving. Please match my signature on Application form, Complain letter and on courier receiving then u will know the fact that sign on courier receiving is not signed by me it’s an forge signature

7) Dear sir it’s my heartiest request to you kindly resolve this case on priority basis and do the needful and revert back as soon as possible.

8) Please take strict action against the company SMC and TATA for misselling and fooling the customers.
