I, SathyaPrabu Rajagopal formerly associate of CTS Chennai (Associate Id.275818) was released from CTS on 05/10/2012 and subsequently sent my PF account closure forms to the HR SS (Provident Fund Team) Payroll Shared Services – Benefits Team Cognizant Techology Solutions India Pvt. Ltd, Perungudi Chennai – 600096 on 12/03/2014 which was acknowledged by CTS vide TCKT ID: BEN00008794 Dt.25/03/2014 remains not processed since the last 70 days inspite of repeated requests made by me.

My PF account closure forms in CTS office need to be sent to PF office expeditiously so as to enable me to get my PF claim amount.
