Cable Operator – No cable refuse to refund amount

We have shifted from one address to another in Bangalore last month, with same cable operator services which we are availing, operator has taken 500 rupees for activation and service but from one month there is no cable service in my home, every time I call the operator he will answer we will check but no action is taken, we have asked him to payback the advance amount(500 Rs) and Digital Box charges(3,500 Rs) for failing to provide service which he is not giving back and refusing to do so, please look into this and take appropriate action.(Note: Please find the attached document where I have shared the current status of TV(cable)even though smart card has been inserted, restarted the set up box done everything but same status)
