Sub: Cash Plus Policy No.03238585 dated 03.10.06

I regret to advise you that captioned insurance policy was taken by me on 03.10.2006 and at the time of
insurance of policy I was advised by your financial advisor (Agent) that H.Y. installment of Rs. 12500/
– was to be deposited for three years and thereafter no premium was to be required to be paid. I deposited
regularly 6 H.Y. Premium without any default. In this regard CCO was also obtained by me from your
office on 29.9/09 and was advised that no further payment is required.

I was astonished to revive your letter dated 16.02.10 along with your cheque no. 529833 for Rs. 15768.91
Advising me for force closure of my above policy. It is a matter of great distress that I deposited Rs.
75000/- and my policy was force closed, and only Rs. 15788-91 was paid to me against full final
settlement.

The policy was forced closed unitarily without giving me any notice/intimation. The Act of ICICI
prudential proved to be cheating activity and rendered me a great critic of ICICI policy and advised my all
friends/relatives not to take any policy from ICICI Prudential and image of prudential has been tarnished in
the market.

I came to understand that premium allocation charges was recovered at a very high rate and this fact was
not brought to my notice at the time of issuing policy by any of your representative.

I had great trust on ICICI Prudential but now I have lost my faith and rendered my self to indulge in anti
publicity of ICICI Prudential. Before this. I have also lodged a complaint soon after received a letter for
force closure of my policy but nothing has been responded from you after lapse of considerable period.

In the end I request to kindly redress my grievances and arrange to refund the remaining balance of
principal amount with interest at least.

This may please be treated as most urgent and action taken in this regard may be intimated to the under
signed.
