With due respect I Uttam Kumar Dutta (LIC Health Plus Member, Member No: LIC 00024139A, Policy No: 455433587, Claim No: 0611469) unfortunately fell on accident on 07/05/2011. The left forearm was seriously injure with multiple cut injury by Glass ([U]Artery, Nerves and tendons were cut into two division[/U]). I have already sent the required documents asked by Medicare TPA Service (I) Pvt Ltd,6, BISHOP LEFROY ROAD, KOLKATA 700 020 on 14/07/2011 to the following address

Recently I made a phone call to JALPAIGURI DIVISIONAL OFFICE, and from there I came to know that they sent the all information asked by Medicare TPA Service (I) Pvt Ltd,6, BISHOP LEFROY ROAD, KOLKATA 700 020, but till now my claim is not paid to me.

Sir; there has been passed a five month time period to process the claimed by me. Please do the necessary steps so that they approve my claim as soon as possible.
