I have applied for addition of name in birth certificate of my daughter at Suwidha Centre Patiala vide Token no.19006, acknowledgement receipt no.15/2014/00389415 dated 11.02.2014. I was given 25/02/2014 as date to provide the service, Now, I checked the Online Status of the application and was shocked to see that the
application is still in pending status. I have to report it on your website as no feedback / complaint box is provided at Suwidha Portal.
