My Name is R.Vijaya and i am a Life Insurance Agent is Exide Life Insurance having agency code EXL 60338672, and Composite Insurance Agent having agency code SHABA00 00253456. While at the time of applying for agency code I have given my Aadhar Card for ID Proof and Address Proof which is in R.Vijaya. Copy of Pan card is given for Commission IT deduction purpose which is in G.Vijaya. Agency code is created as G.Vijaya in stead of R.Vijaya. Cheques also issued towards Insurance Commission in the name G.Vijaya, whereas I am having my Bank Account in the name R.Vijaya, as a result not able to deposit the cheques. After having this issue, I have applied for Pan Card Correction. Kindly make a note in this regard and change my agency code in the name R.Vijaya. Kindly do the needful at an early date.
