Sub:- Complain against the HDFC Life Insurance Co. Ltd for gave the wrong policy plan (HDFC SL Classic Assure Insurance Plan) in place of my proposal plan (HDFC Sampoorn Samridhi Insurance Plan) and wrong details filled in my insurance kit.

Sir,
With respected in reference to above subject that the HDFC Sampoorn Samridhi Insurance Plan discuss on mobile By Mr. Omprakash & Gorav (Rignal Manager HDFC Jaipur Mobile No.09529924796 on dtd. 30.08.12. Then they send HDFC representative Mr. Sunil (Mobile No. 7665329769) from Bhilwara branch on dtd. 31.08.12, I filled my proposal form in my hand writing and given required decoments with plan selection in presence of HDFC Life agent. But when I received my insurance kit on dtd. 25.09.12; I surprised that my policy plan changed and gave wrong policy plan (HDFC SL Classic Assure Insurance Plan) in place of my proposal plan (HDFC Sampoorn Samridhi Insurance Plan) and find following mistake in this kit.
1:-The proposal form is not in my hand writing it is filled by another person details is try to making same.
2:-My Photo is change.It’s not my Photo in given kit.
3:- In form my mobile nos. also filled wrong (9413332395) while I filled my correct mobile no.9413382395 in proposal plan.
4:-The Address also changes .Its not matched which I filled in proposal.
5:-The signature also change .It’s not my signature but try making same by HDFC life it’s totally 420case.
6:-I also filled my E-Mail address (rajendrak.sharma@yahoo.co.in) but it’s not filled in this policy kit.
Sir due to find so many mistake and change the proposal plan. I complaned against the HDFC Life (Mr. Gorav-Jaipur Branch, Mr. Sunil-Bhilwara Branch ,Mrs.Metilda Stnley Sr.Vice President –Customer Relations HDFC Life Jaipur and agent Probus Insurance broker Ltd. Mumbai).
Kindly cancelled the above policy plan and arrange for return my Rs. 30,000 early as possible in my A/C No. 61016269223 SBBJ Bank Chittorgarh with compensation and Idecleared that I will not accept further any plan by HDFC Life.Kindly give me any gideline about this.
