Renewal of car policy, car registration number GJ 27 C 6614, Expiration date of policy at the time of online generation of policy quote at ICICI Lombard web site 20 May 2013 (ENCLOSURE -1) 2. We generated online quote on 30 April 2013 (ENCLOSURE -2) 3. We sent Cheque bearing number 706316 sent to ICICI Lombard on 30 April 2013 and was delivered at address on 2 may 2013 (ENCLOSURE -3) 4. Cheque was cleared from our bank account on 9 May 2013 as per bank statement (ENCLOSURE -4) 5. We sent mail to ICICI Lombard on 22 May 2013 inquiring about status of policy. (ENCLOSURE -5) 6. We received return mail quoting reference number ICICILom:2013052200208 to our complaint. Asking certain details on 23 May 2013. (ENCLOSURE -6) 7. Said mail was replied with relevant details on 23 May 2013 (ENCLOSURE -7) 8. Received mail from ICICI Lombard stating that policy document would be processed by 7th June 2013 (ENCLOSURE -8) 9. Later for whole May we did not received any communication. 10. We received call from ICICI Lombard to undersigned around 13th May (Exact date is not recallable as undersigned was out of station and was touring) stating our policy proposal is rejected and we will have to approach ICICI Lombard for refund of payment. Tele calling agent refused to cite any reason for rejection , as he was not authorized to give any reasons. 11. Today, i.e. June 19, 2013, I logged on to ICICI website and found that our policy is rejected, we are unable to find any reason of rejection though. (ENCLOSURE -9) 12. Contact number given in their mail 022-39803680 or Mr. Mr. Sachin Chakravarti is not replied. Appears to be fake number given for the sake of it.

Immediate issue of policy so that our policy do not have any break and financial compensation of Rupee 200 000 caused due to mental harassment and also cover potential loss due to non issue of car insurance policy.
