Respected sir/Madam, I would like to invite your attention that I was agent in “ICICI PRUDENTIAL LIFE INSURANCE“at Durgapur Branch in West Bengal. Since 30/05/2014. I have given application of Noc at Icici prudential life insurance(Durgapur Branch) at 09/09/2016.my Irda Urn- ICII2005140076, Licence no- 10049765,agent id-WBU310514LIICI751003 & my pan no-ACBPF4719J. now I am working with other company. it’s very urgent otherwise I can loss my job. All formalities is completed by me for Noc.

I have made multiple visit and reason everytime that head office does not respond. Noc Certificate (icici prudential life insurance) issue by company at Durgapur Chief branch manager.but my pan no is not detach till now. There for you are requesting to please release my pan no.
