LIC does not send the Health Card of Health Plus Policy even after one year

Dear Sir,
I had taken the LIC Health Plus policy last year on August 3, 2009 through a LIC agent Deonath Shukla, Minalta Investment Agency who belongs to Bhelupur branch, Varanasi, U.P. My LIC Health Plus policy No. is 286576810. As per the rule of policy, the LIC is supposed to send a Health Card mentioning the names of the insured members of the family immediately after one takes policy.

But, I have not received the Health Card of that policy even after one year. I had sent an E mail to mm.varanasi@licindia.com on Fri, Jul 16, 2010 at 12:16 PM ( the Customer Care E mail ID of my area which is mentioned in the Website of LIC) to resolve the problem but they have not sent any reply so far. I have requested to my agent Deonath Shukla many times but he did nothing till now.
