Policy Number : 834264851 and 834150933 in favour of Manas Siddharth Patel. 3 , Kailas Kunj Society, Behind Lal Bungalow, Ellisbridge, Ahmedabad. 380006 Mobile No. 09825443223 We have already paid Two Years and Nine Months Premium. The mentioned both policies are Lapsed. Reason is not paid the premium.

Our Request : If L. I. C. accept one more premium so the policy complete full three years premium and in future as per L.I.C. rules we get some reasonable refund amount. Thanks, Manas Patel
