This is to inform you that LPG (Indane) cylinder was delivered to consumer ID 37500 00006 6186089 on 10.10.2016 but did not receive the subsidy till date. Details of booking and delivery is given in attachment file.

Subsidy last received in the month of July 2016. You are requested to resolve the matter immediately.
