I am Sankar Mookerjee, was working as an agent in HDFC Life Insurance Co. Ltd. since 14th July 2010 (Code:- 419109) (Pan No. ARDPM6665R).

I was submitted my resignation on 25.09.2013 and I was issued necessary Cessation Certificate on 04.04.2017 from HDFC Life Insurance Co. Ltd. Cessation Certificate attached herewith for your reference. But still my pan No. is visible in IRDA portal.

Therefore, you are requested to remove my Pan No. from IRDA portal as soon as possible and oblige me.
