I am a serving ARMY OFFICER.I took a DEFENCE SALARY PACKAGE(DSP)account in the above mentioned branch. MY ACCOUNT No. is 32748212736 and CIF NO is 86568158020.It is a joint account with my wife.Date of ACCOUNT OPENING IS 22 JAN 2013. I have not received personalized Multi City Cheque Books and personalized Ordinary Cheque Books and a spare ATM CUM DEBIT CARD.Which is a part of account opening procedure.Only passbook and one ATM CUM DEBIT CARD has been received so far.I have been to the branch many a times but I was told that it will take some time because it will come in a new format from MUMBAI.

SIR IT’s a request from my side that i should be issued with the cheque books and a spare atm cum debit card which is authorised to a DEFENCE SALARY PACKAGE(DSP) account holder as an OFFICER. SIR I HAVE COMPLETED AAL THE DOCUMENTATION WITH THE BRANCH BEFORE OPENING THE ACCOUNT.I have verbally interacted with the bank staff.
