Tata Sky Fault not repaired, charged twice

I have a Tatasky connection for last 3 years almost with subscriber ID: 1061694541. And I always recharge my Tatasky a/c with Rs.300 (or above) per month. From the beginning of April, 2013 an error message appears on the screen with Error code 02: “to reinsert the digicard” only after 1-2 hrs of TV viewing. And the problem was not regular. So I complained through the customer care and the complaint no. was SR0404130957217 on 04/04/2013, and the resolution was given verbally to reinsert the digicard after gently clean with smooth clothes. But that did not help. So I again complaint on 12/04/2013 no. was SR1204130962431. And an ‘installer’ visited my premise on 14/04/2013, and tried to resolute the matter by ‘dish tracking’, and verbally told that the problem was resolved. And I had to sign their paper to satisfy their needs. As the problem occurs not in regular, and in the installer’s presence the problem was not appeared. And he verbally told me if the problem continues they would come again and resolve and I was charged Rs.150/- from my Tatasky a/c. And just after the installer’s visit the same problem occurred and I reported the matter to the customer care, no. was SR1404130963856 which was closed without any steps taken and I had to again complain on the same day, no. was SR1404130963919. According to the problem another installer visited my premise on 16/04/2013 and he too failed to resolve the problem, and just after the installer’s visit the problem was reappeared. Here to specify that the customer care executive told me (on telephone, no. 18001806633 on 16/04/2013 at about 9:30 pm and on the same telephone on 18/04/2013 at about 4:10 pm) no charges would be deducted for that 2nd visit, and it was their practice to resolve a particular problem with a single one time deduction from Tatasky a/c of Rs.150/-. But for this 2nd visit again Tatasky charged Rs.150/- from my a/c. So, without resolving the problem Tatasky charged me Rs.300/- in total. So, without resolving the problem Tatasky charged me Rs.300/- (Rs.150/- X 2) form my Tatasky a/c. Still I am having the problem with my a/c balance made zero.

I want the the total money (Rs.300/- + Rs.10/-) back in my a/c and a fine of Rs.500/- for the interruption on TV viewing each day. (The Tatasky system should be fined Rs.500/- each day of interruption of service until they will repair the fault and resume services) As I wasted my valuable time behind requesting Tatasky associates day after day, minute after minute.
