United India Insurance Company Limited – company has denied my Cashless treatment without any proper Reason

Dear Sir/Madam, We have Medical Insurance of United India Insurance Company Limited through E-Meditek since 4 years. Inspite of paying it reguarly during my father’s hospitalization company has denied my Cashless treatment without any proper Reason. My father (Radhakrishna) has been admitted in Kamineni Hospital and i applied for Cashless treatment for a neuro surgery.He fell down from vehicle on 12th Nov 2012 and came back home with minor injuries which we had first aid at home and never consulted any doctor. Later after 1 month on 8th Dec he started complaining about headache which made us to consult a doctor and report showed us blood clot in brain. But when i applied for cashless treatment E-Meditek was asking for proof of patient not consumed Alcohol on 12th Nov. We have even provided a written statement from treating doctor saying that the patient is non-alcholic. But Insurance company want proof of 12-nov, which is baseless as we never consulted any doctor. Please check and let me know what need to be done w.r.t to this issue. Let me know which all documents you require as proof so that i can issue a complaint on E-Meditek for not approving my approval in time and making me suffer mentally for money.
