I am sunita rani . Sir i want to inform I am applying for the addition of the name of my new born baby birth certificate three time from suvedha center jalandhar as on 02/12/2016 . When I am visit in suvidha centre for receiving the birth certificate then such sewa kender executive tells every time to resubmit all the docoments again and this is happen all the three time and the online record also show the same status. .Infact i Have to submit all the documents already in first time. I have a slip of date of birth which is issued by the municipal corporation jalandhar. but in that slip the name of the baby is not added by them and now we want to add the name of the baby in birth certificate. please tell me what are the problem ocuured for the addition of the name of the baby. so i humbly requseted to you for provide me a Birth certificate as soon as possible. so I can apply for the future benefits of baby. name of baby- Vansh gularia ack. no- 016091001/397/021216
