I am Mr.M.Satyanarayana, worked as an advisor of Birla Sun Life Insurance at Tenali, Guntur Dt., Andhra Pradesh branch in the year 2008. My agency ID is “BQ1253”. My PAN.No. is “BKAP2930J”. My licence No. is “4141949”. I stopped business in your company many years back and subsequently my agency is expired in your company. Now I intended to join as an agent in Life Insurance Corporation of India and according to LIC instructions, I applied for NOC at your Vijayawada, Andhra Pradesh branch which was attached to this mail on 14-12-2016. I lost my ID card and appointment letter and customer details so many years back unfortunately. I mentioned those points in my resignation cum NOC requisition letter. But till to date I did not received NOC from you. I get in touch with Vijayawada branch so many times on phone call. They told no reply from head office. Kindly arrange me NOC on urgent basis.
