Canara Bank TDS Certificate Not Received

I am a Senior Citizen, aged 80, customer of Rohini branch of Canara Bank.

My customer ID is 11153521.A sum of Rs. 780.00 and Rs.745.00 was deducted from maturity amount of my FDs. maturing on 19th Sep. 13 and 18th Oct. 2013 respectively. However, I am yet to receive TDS certificate for the same. This is not reflected in Form 26AS as well.

I am not able to get the credit of tax deducted and finalize my ITR. Since I am presently staying with my children, the certificate may kindly be sent to the following address:
H-120, Manoj Vihar, Indirapuram, Ghaziabad-201014.

This address may be updated in the records for all future communications as well.
