Dear Sir/Madam ,

I Shanti Lata Routray have got the agent code of Kotak Mahindra Old Mutual Life Insurance bearing agent code no-KLI60279101(effected from 1st April 2015) & IRDA License No-882-10416808. I have not done any business in this code. Now I am preparing for some exams, so I don’t want to continue my code with Kotak Mahindra Old Mutual Life Insurance Company.I have gone to the Bhuabneswar,Odisha Branch office for NOC .But they told me to generate a new agent code & also given the forms to open & asking for Rs 5000/-& then only you will be eligible to get the NOC within 3 months .I am not satisfied with their answer. So I request to you please remove my PAN no form portal.
