Dish TV – STB gives error of Singal

I have purchased Dishtv standard set top box online on their website dishtv.in in November 2013 under their diwali offer, while taking the offer executive committed me following 3 free addons: 1. 3 months free access to Super Platium Package (I got that one fine). 2. Cash of Rs. 1500 on various recharges being made in next 30 months (I doubt on this and contacted them several times via phone). 3. Gift vouchers from various online sites worth Rs. 1500 (sites like homeshop18.com, flipkart.com etc.) – I never got them. Apart from that the day I got it installed, I could never got it working perfectly, after every few days STB gives error of Singal not found or Card not inserted etc, I have reported these things initially on phone, they sent someone and he corrected but again it was happening after every few day, then I have written a detailed email to them on 11th April (Attached pdf contains mail communication details), you can read in emails that they have clearly denied of giving financial account statement, which is my right, and even today when I called them for the same error, the executive asked that it will be chargeable service.

I want them to take their device back and refund all the money I paid to them, though there is lot of mental harassment and loss of time in various phone and email conversation, but I am not going to fight for that till now, but if they deny from paying my money then I will surely take this matter to next level.
