HDFC Life Insurance – Asking to Submit documents

I applied for HDFC’s term insurance policyClick 2 Protect Plus‘ by making the payment of Rs. 12,217 on 23rd Mar 2015 through policy bazaar (Application No. S299999810673 and Proposal No. 17536607). I was asked to take certain medical tests for completion of the policy, which I took on April 4th. After this I started receiving emails saying there are further requirements on my proposal eg. certain questions are not answered. The link provided in these emails takes me to the ‘Upload your documents’ page of the HDFC Life website. On this page, in the KYC section, Q7 is ‘Addendum POS’. In the drop-down of this question, there is only one option – ‘Addendum_POS’ and option of uploading upto 3 documents. Same story is with question 8 – ‘MID’. I had to call customer center to confirm that there is nothing pending from my side. I was asked to take the print-out of the proposal form, which I have never received from HDFC Life, write the answers manually, scan the documents and send it to HDFC Life. Till date I have not received the proposal form from HDFC. I tried explaining this multiple times to the customer care executives, promises were made to send the application form at the earliest each time, however haven’t yet received the application form, but been receiving emails again and again asking to submit the documents.

I just want HDFC to send me the proposal form, so that I can provide the requested details and then hopefully HDFC can close my policy or refund the amount that I paid!
