Kotak Life Insurance – forgery sign verification not done

I had taken a Life Insurance policy of Kotak. I later cancelled it within 15 days and send a request personally. I have a proof of me visiting the branch. 10 Days later I receive a mail thanking me for retaining the policy. On Calling Kotak I get a reply that they have a letter signed from me saying I want to retain the policy. I have signed no such letter. This is a case of forgery. While cancelling the policy they called me for verification. Why was this verification not done while retaining the policy. How did they allow any person without documents to sign the policy in my name? Why was this verification not done? I had brought this from Amit Shah of Reliance securities and he has forged my signature.

I want the money back with the penalty of forging my signature. I want Rs. 100000 as a penalty from the bank for accepting the forged letter in my name without the verification.
