MTS MBlaze Broadband Speed Issues

I had issues from the day one, I was getting minimum speed, raised so many complaints and the complaints were not resolved. I have attached the email conversation that i have had with MTS Broadband Team. One point of time I gave up and called mts to inform that I want to cancel the service. I used the service till august 2014 and the service kept getting worst and worst. The account was deactivated as i refused to pay bills. I told them i want to cancel the service as it didnt not work properly and i was informed that i have to clear the bills to cancel the service. I paid off my bills and thought they cancelled the service. All of a sudden i got a bill on november for the service which i ve never used called mts to cancel the service, they have created a waver and they never took a request of cancellation, im keep struggling with this ppl for almost 4 months now and when ever i call them the only answer i get is that, they have to process the waver and then the service will be cancelled. however no one ever gave me any waver number, i kept insisting on getting a number, but they said they cannot provide me with one. Now i have a legal notice saying that I have to pay the bills for the service which i have not used. Im trying to fix this issue from 10.1.2015 again, and i m still getting no where.

I want the bills to be waved off and need to service to be cancelled, and want them to pay the money for the phone calls that i made to MTS
