With due respect I am informing you that I, Ashoka Dey, Daughter of Tarak Nath Mallick (PAN No. ADTPD8247L) ex-teacher, 63 years old & bad health lady. I am suffring so many disease like asthma, Sugar, Liver disease for those diasease I spend large amount of money for doctor consultation, pathology, medicine. There was a some fixed deposited amount at State Bank of India, Domjur Branch, when I was went to bank to collect my money after it’s maturity, Bank authority issued a letter & informed me that Rs. 3727/- has been deducted from the matured amount for TDS. The letter & ITR-V application form was filled up & submitted to Office of the Deputy Commisioner Circle-22, Kolkata on 30.03.2012 (Acknowledgement No. 2200005550) for TDS return to my account. But unfortunately till today, I have not received the said detucted amount. Please help me to received the amount as early as possible. I am giving my bank A/c details for do the needfull.
