Dear Sir, I was the agent of Oriental Insurance company Gulabpura Bhilwara (Rajathan) Branch Since 31st May 2016. I got monthly incentive card as well as payment for the month of May 2016. I got monthly incentive card of Rs. 34035 for the month of April 2016. The Gift card no. 4573925218886510 of ICICI Bank dated 18th July2016 for the same.
