Vodafone – Fraud of documents and disownership of my number

t is matter of high escalation and want an immediate help on the same. I was using a number #999 999 4xxx with SIM no. 7002569xxx since last for more than 4 years. On Apr4th, 2014 someone put a fraud request for SIM replacement and then for MNP to other subscriber without my knowledge. The services for my number stopped suddenly, first I contacted to customer care. They suggested to visit the Vodafone store & then I visit to Dwarka Vodafone store and discussed the whole matter. The store supervisor confirmed that this fraud request would be cancelled and then issued another SIM #82023919xxx to me and converted it into postpaid and assured to activate by next day. But MNP could not cancelled and number went to Airtel and then I escalated to higher management and number came back after 15 days. This incident happened second time and now I want to lodge a FIR in police station and visited multiple times in Voda store but I got the number back and Vodafone not starting my outgoing services.

I want my services back. Punishment to fraud customer with legal process. And economic benefits because of harassment, moving to different locations like Voda store, police station, mini store, balance lost, etc.
