\I have admitted my father AJIT KUMAR MUKHERJEE at ZENITH SUPER SPECIALIST HOSPITAL, Kolkata – 700056 on 18.12.2013 due to Breathing Trouble in ICCU Registration No. – 379786 under Dr. TARUN CHAKRABORTY. After several medical treatment & testing his health condition was improved and he was shifted to General Ward on 21.12.2013. As per the advice of doctor endoscopy of the patient was done on 23.12.2013 in the hospital, at that time his health condition was quite good. from the evening of 23.12.2013 again breathing trouble was started along with vomiting with blood, due to this he was shifted to Ventilation, ultimately we lost him on 24.12.2013 at 02.45 AM. We assume that due to endoscopy there may be some malfunction in his health. We require your assistance for this case.
