ICICI Prulife Matured Amount Not Received

1. My policy matured on 1/12/2013 2. At the time of last premium payment I visited ICICI Prudential and asked for option form for full payment, Employee of company told me they will send forms at time of maturity 3. In the month of October I received from ICICI Prudential annuity quotation and bank detail forms through courier 4. ICICI Prudential avoided to send surrender of policy form of full maturity amount purposely 5. I duly filled bank detail form and send to ICICI Prudential through speed post, presuming that It is only form for full maturity amount and rejected pension option by not submitting Pension forms I therefore complained to ICICI Prudential as my maturity amount was not credited in my bank account as below 1.As per para 10.3 of policy I can opt for full maturity amount, Why ICICI Prudential has not sent option required form for full maturity through courier? 2. Why I should I physically present in office of company for full maturity option as required by them 3. How & Why ICICI Prudential close my option that I cannot opt for full matured amount after 3/12/2013? 4. Mumbai Ombudsman of company refuses to entertain complaint 5. ICICI Prudential blames IRDA as IRDA has approved the procedure of company. It is surprise how IRDA approves the procedures of company against terms and condition of policy with disadvantage to policy holders by deleting option of full matured amount available in policy and benefit to company. Due to IRDA company is still not ready to pay our matured amount

ICICI Policy no. 00576862 matured on 1/12/2013. As per clause 10.3 of policy we opted for matured amount. Over one year passed we still not received from ICICI Prudential. Help required to get matured amount from ICICI Prudential
