SIR/MADAM, I had ordered for testing of water samples and paid the prescribed fees of RS 380/- to REGIONAL PEOPLE HEALTH DEVELOPMENT SOCIETY GUNTUR on 12/07/2016 but they didn’t send me any report about water samples till now and when I telephoned them there are giving vague replies.

Kindly ensure me that the reports are furnished to me at the earliest and also provide us the compensastion for not proving the reports on time i.e., with in 10 days as per their citizen charter
