Sir My Husband Late Mr. Ali Akhtar died on 25.2.2011. His saving account was there in PNB Kumarganj Faizabad (UP) 224229. His saving account No. 0659000400033xxx and Balance of Rs. 4554 (Four thousand Five Hundred fifty Four only). I have submitted to following documents for claim. 1. Claimant Form 2. Death Certificate 3. Notary By Magistrate 4. Rs 100 Blank Stamp 5. Rs 10 stamp signed by the children (minor) 6. Relation Certificate (By Tahseeldaar) 7. Two witness of same bank branch 8. Ration Card (Xerox copy) These are the documents which i submitted. But he is asking for copy of Parivaar Register (Kutumb Register) which is not allotted for urban areas as I reside in Sultanpur City 228001 not the village. In urban areas only ration card is allotted. (In UP). But he not paying heed to this issue and says he can not give claim without Parivaar Register.

I want my claim settled as soon as possible. as nearly Rs 8000 Spend on journey I want them back and the extreme physical and mental stress which can’t be described.
