My Saving account no is 30404626764 in makronia branch of SBI bank. I opened my saving account in this branch in june- 2008 and in march 2009 amount of 680 has been deducted from my account. Then I go to branch and asking for this deduction then branch employee said fund deducted of SBI LIFE Insurance Policy. Policy No- 86000052602. Then I complaint that this is not my policy and I never got any premium slip or any document from SbiLife Insurance. After 3 months of filing a complaint I got my 676 amount credited back in my account. In 2010 again amount of 676 has been debited from my account two times. Then again in 2011, 2012,2013,2014 one times every year. Total 6 times my account has been debited with amount 676 to 680 year each. I have given 5 times complaint letter to SBI Makronia Branch but I have never got any satisfied response. Then I go to SbiLife office and ask him to duplicate copy of my policy which I never had. Then I got duplicate copy of Policy. In the policy only account number and name is mine but ” Address, Date Of Birth , Nominee ” are not mine. For all Detail plz find attachments. From 2008 to 2010 I lived in Jabalpur then 2011 to 2012 I lived in Delhi/Pune and from 2012 to 2014 i lived in Mumbai and it was not possible that every time I spend money and give time to go to Branch and asking for refunding my amount and close the policy but I do all the things.

Please close the policy from my account and refund the amount with all type of amount I have spend for this. Total amount spend in such all thing is 70000 approximately. Attachment 2 named “sbilife app 1”- 5th complaint letter given to SBI branch makronia. Attachment 3 named “sbilife app 2”- Application given to SBILIFE asking for duplicate copy and close the policy. Attchment 4 named “Account Detail”- My account details which opened in SBI Makronia Branch.
